HomeMy WebLinkAboutResolution 76-1997 RESOLUTION NO. 76-97 RESOLUTION APPROVING HEATH INSURANCE PLAN AMENDMENT NO. 1 AND NO. 2 WHEREAS, resolution No. 109-96 established a self-funded program for the provision of health insurance; and WHEREAS, a Healthcare Plan was adopted which establishes the terms, provisions, and conditions of the health insurance plan; and WHEREAS, a Healthcare Benefit Plan Booklet was established to summarize the benefits available to participants in the health insurance plan; and WHEREAS, the attached Plan Amendment No. 1 and Plan Amendment No. 2 change the language in the Plan document and the Benefit Plan Booklet. NOW, THEREFORE, BE IT RESOLVED by the Brookings City Commission that the Health Insurance Plan Amendment No. 1 and No. 2 be hereby approved.
