RESOLUTION APPROVING HEALTH INSURANCE PLAN AMENDMENT NO. 3
WHEREAS, resolution No. 109-96 established a self-funded program for the provision of
health insurance; and
WHEREAS, a Healthcare Plan was adopted which establishes the terms,provisions, and
conditions of the health insurance plan; and
WHEREAS, a Healthcare Benefit Plan Booklet was established to summarize the benefits
available to participants in the health insurance plan; and
WHEREAS, the attached Plan Amendment No. 3 change the language in the Plan document
and the Benefit Plan Booklet.
NOW, THEREFORE, BE IT RESOLVED by the Brookings City Commission that the Health
Insurance Plan Amendment No. 3 be hereby approved.

PLAN AMENDMENT NO. 3
The City of Brookings of Brookings, South Dakota hereby amends its Healthcare Pian adopted
January 1, 1997, with such amendment being effective January 1, 1998 at 12:01 a.m. standard
time.
The Plan shali be amended as follows:
In the Schedule of Benefits, the Mental Health Benefit shall be replaced with the following
language:
Mental Health Services (See Section 4.1 T.)
'Note: This per�centage will never increase fo 100%, or accumulate toward satisfaction of the Out-0f-
Pocket limit.
➢Psychological Care
Inpatient Services Subject to deductible and coinsurance. Calendar year maximum:
30 days.
Outpatient Services Subject to deductible, then paid at'S0%. Calendar year
maximum: 30 visits.
➢ Drug Dependency or
Alcoholism
Inpatient Services for Drug Subject to deductible and coinsurance. Limited to 30 days during
Dependency 6 consecutive months. Lifetime maximum: 90 days.
Inpatienf Services for Subject to deductible and coinsurance. Limited to 30 days during
Alcohol Dependency 6 consecutive months. Lifetime maximum: 90 days.
Outpatient Services Not covered by this Plan.
The balance of the Schedule of Benefits shall remain as previously adopted.
