INFORMATIONAL ITEMS Merkley shared two articles of interest; “Transparency Takes Off - Payers and patients want prices and some hospitals are making it happen” and “Feds target doc-owned device distributors – Hospitals at risk of patient, kickback complaints when using PODs”. REPORTS Operational CEO Report Merkley brought attention to the new Inspiring Local Health Peer Recognition program that will be rolled out to all staff in October. The Hospital wide program was developed to provide a way for staff to recognize and nominate their peers within the health system who go above and beyond in relation to BHS’s five pillars of success: Finance, Growth, People, Quality, and Service. An internal reward and recognition motivation assessment tool has also been developed and allows the program to be employee specific and to better understand how each employee prefers to be recognized/rewarded. 4 Wosje made an inquiry regarding the new ambulance campaign and why there is a need to have a fleet of five. Merkley noted that the new Type III ambulance will be equipped to provide (ALS) Advance Life Support and will replace an older ambulance. ALS provides a higher level of care that requires special training in monitoring heart and airway management and other traumas. There are several reasons why the Brookings Community needs 5; one unit is on standby with supplies in case of a major catastrophe. The other four are used for 911 emergencies, transfers to Sioux Falls or Mayo, standby for athletic events at SDSU and Brookings High School, etc. At least one ambulance is always running; the ambulance crew averages five (5) calls per day. An ambulance does not have a long lasting span due to the average mileage of over 100,000 miles per year per vehicle. It was noted that each unit is self-supported through Brookings Health System. BHS does not receive any City or County tax revenue for the fleet of vehicles or operations. Baker asked how extensive the chemotherapy program is at BHS. Hillestad noted that the Out Patient area was designed with 5 chairs to provide chemotherapy treatment. The nurses take specialized training to be able to infuse chemotherapy. BHS’s pharmacy department is also equipped with the training to mix the drugs which includes mixing for the Avera Clinic. The clinic has two chairs to provide chemo. BHS has a very good relationship with the oncologists in Sioux Falls; however, there is still a challenge in breaking the cycle of migration to Sioux Falls. Unless the patient knows we offer it locally, they won’t inquire about having it done at Brookings Health System. BHS has been offering chemo for at least the past 6 or 7 years. BHS provides time and cost savings as well as convenience. CFO Report In addition to her report VanMeveren reviewed the BHS financials for August. The majority of the variance came from clinic operations. Gross revenue and volumes were up, but contractuals are negatively impacting the additional revenue. VanMeveren is working together with the Clinic on a plan to convey unbudgeted variance reports to the Finance Committee. Many good projects are going on and Capital and Operational budgets are underway. Englund noted that NBV had a YTD budgeted loss of $600k and the YTD actual loss is only $289k. Hillestad noted that NBV is focusing on rehab therapy to maximize the acute level of therapies which provides additional net revenue. Baker inquired about the Data Analytics Taskforce. VanMeveren shared that gathering data is vital in order to understand what is going on at BHS and being able to put the data in a format that is user friendly and easily summarized. CNO Report Hillestad made note of the ED nurses who are certified in SANE (Sexual Assault Nurse Examiner) training. 5 The nurses are certified to provide comprehensive care to sexual assault victims, collect evidence, and provide compassion and counseling. BHS currently has 3 SANE nurses and will soon have 5. 4 RN’s recently completed training in PICC (Peripherally Inserted Central Catheter) insertion. A PICC is generally ordered for chemotherapy patients to provide intravenous medication for a longer period of time. Hillestad also noted the record number of 37 births in the month of August. SDSU nursing students are in the facility and have begun their clinical training. BHS provides a great opportunity for onsite teaching. Board Committees Building Committee: The Building Committee met on September 22, 2014. The committee discussed the RFQ process, architects/engineers, and meetings with the City Councilors. There were no actions items. Finance Committee: The Finance Committee met on September 23, 2014. The committee reviewed and approved the payables, adjustments, and financials for August. Also approved was the resolution to cancel certain outstanding checks. There were no action items. Quality Committee: The Quality Committee met on September 22, 2014. The Committee reviewed the Customer Experience Report, Performance Improvement Project Update, and Safety and Ergonomic update. There were no action items. Strategic Planning Committee: The Strategic Planning Committee met on September 22, 2014. The topics of discussion included the Hospital expansion, State Health Plan, Capital Campaign, and other various topics. There were no action items. UNFINISHED BUSINESS State Health Plan – Tier 1 Merkley noted that BHS is exploring a way to match out-of-pocket costs and hold State Plan members harmless from the benefit differential between Tier I and BHS. This is a very difficult process due to the fact that we are dealing with two business offices; the Clinic and BHS. Dusty Johnson has requested a meeting on October 3rd. RCH Demo Program - Update Merkley mentioned that he received positive feedback from our congressional leaders during his recent visit to DC. As mentioned before, this is a very important issue for BHS. The program has allowed BHS to offer a level of service that it would not have been financially able to do otherwise. Merkley will be working on this while finding ways to continually educate congressional leaders on its importance. NEW BUSINESS BHS BOT Bylaws 6 Merkley noted that the Bylaws required an update due to the change of the BHS Mission Statement. Other slight modifications were made to correct spelling and formatting errors. A motion from Booher, second by Beireis to approve the Bylaws as updated. Action: All present voted yes; motion carried. AHA Small or Rural Hospital Council Merkley thanked the Board for their support to be able to attend various AHA Council meetings. Merkley recently attended the AHA Governing Council meeting and noted it was very informative to listen to others from across the nation, to hear ideas, and learn about the challenges others are facing in the health care industry. The challenges are going on nationwide; from payment reform, physician recruitment, to outcomes and quality. AHA is a resource we can use to help us. SD is very much behind other states when it comes to Medicaid expansion. SDAHO is South Dakota’s voice on Medicaid expansion. Merkley will share information on Medicaid expansion as we move further into the legislative session. STRATEGIC DISCUSSION Jamie Orlikoff Presentation Follow up Consensus amongst the Board was that overall it was a good discussion and many members expressed value in the presentation. The content was interesting and provided a good background of where the health care industry is heading. Getting mentioned that it was very thought provoking. Merkley noted that supply chain management is critical. We need to do a better job of analyzing our options and be able to purchase supplies at a more reasonable cost. Our organization needs to put the time and analysis in to area such as standardization, lean, value analysis, and understanding our costs. Many of these topics will be on our agendas for future discussion. Baker inquired if any Board member had any questions or concerns regarding the Debt Capacity Study. It is imperative as a full Board that each member understands the specifics associated within the Debt Capacity Study. Merkley mentioned that he and Baker will be conducting meetings with the City Council to provide more details regarding the Debt Capacity Study and the building project.
