Establishing Medical Insaraace MoQthly Contribation Rates For
The City Of Brookings Health Insuranee Program
WHEREAS, Resolution No. 109-9b established a self-funded program for the provision of health insurance
for City emptoyees; and
WHEREAS, establishing a corridor fund for sharing of savings and risk for a specified claimant allows
savings to the Health Plan; and
WHEREAS, the Brookings City Council intends to establish the levels of employe;e and employer
contribution to fund the program;
NOW,THEREFORE,BE IT RESOLVED,that the 2004 employee and employer contributions to the self-
funded health insurance program be established as follows:
